Was wondering if the following would be possible within forms:

When a Form is being submitted by a user that goes to a Team, I see there are quite a few columns you can add to be displayed as per below:

Was wondering if there would be a possibility of adding a Notes field in there as well? It can be a field we can tick to be included from a metadata multiline field? or just another field that can be edited after the process has been submitted to a team. 

Will save a lot of effort for a team instead of having to open every process individually and read notes? 

Thoughts?
