Error Saving to Repository

I have a workflow that saves to the repository on Rejection / Approval. The rejection path is working fine with no errors, but the approval path is giving an error and not saving to the repository. It was working fine a week ago but now has stopped working.

The message is:

[LFF5426-ErrorExecuteSTR]

That's not necessary, this is a Forms error.

I guess she had to change a parameter in the form and that had an impact in the workflow.

We were able to fix it! We were previously using version 10.1 which would allow for the form process to work without using Gateways. But with the 10.3 update we had to use a gateway anytime a decision was made (it seems). It is now working using an inclusive gateway at the last step. I'm not sure that it is 100% correct though..but it is working now.
