Hi Andrew,

I would recommend adding Forms' built-in Address, Email, and Single Line (for phone number) fields to a collection on the form, which will logically group them all and allow users to add more instances of the collection as needed--in other works, a set of address fields will always be accompanied by fields for email and phone number.

I recommend the approach above because it would accomplish what you want and you might run into some difficulties passing those values (ie, from the customized Address field type) into Workflow, if that will be part of the process.
