Web Access: Show the "Use Windows Authentication" checkbox on the Login screen

How do you enable the check box option Windows Authentication for the login page in Web Access 10.2?  We have a test server and a production server for Web Access and both point to the same repository.  The test server requires authentication in the browser using forms credentials before the page will be viewed.  Once those credentials are successfully entered the Web Access login page is displayed.  Only the production site has the option to use Windows authentication.  The documentation for Web Access 9.1 says to “select Show the "Use Windows Authentication" checkbox on the Login screen to display it on the Login Page.”  We have compared the configuration site for both login sites and they have the same settings.  Windows authentication is enabled on both sites.  Where is the setting to enable the Windows authentication option to be displayed?

The check box has been removed in the UI in version 10 but can be manually configured in the config file.  Details of what to configure are not stated.
