I am getting the error "One or more required fields were omitted or had a blank value. [9066]" on a workflow that has been published for months now and just started throwing this error yesterday. It continues to do everything it is supposed to do.

See attached screenshots for more details. Any pointers to the root cause will be much appreciated.

Has anything changed in the template security?

Nothing has changed with the security of the fields. In fact they are not used in any templates. Their values are joined and used in a template field however there has been no security change on that template.
