What is the easiest way to have a fill lin the blank field like this:

I have this:

but I'm trying to make it look a bit more like the original.

While it is possible to do this with a heap of css coding. Is there any reason your laserfiche form has to mimic the original?

Could you design it as a form and then using a workflow and a update word document task or fill pdf task to fill in the original document? That way you can use a simpler style of form but still have the required output?
