Workflow - Fill Out PDF Form - Not Checking All Boxes

I have a workflow that fills out a PDF form and emails it to me. There are multiple check boxes on this PDF form and they are all named differently. I built the form from scratch using Adobe Acrobat.

The odd part is that not all of the check boxes work. Most do, but a few do not. I have replaced the template file in LF and also the Attachment file for the activity in Workflow but still no success.

I am using Workflow 10.2.

I have tried using a token value for "checked" and I've also tried telling the activity to set the value to "Checked" via the drop down. Once again, other boxes work and the ones that don't were built the same way in Acrobat.

They are not locked on the template and have no special characters in the names.

Does anyone have any ideas on how to fix this issue?

I'm not sure why, as i did this multiple times yesterday, but I recreated the checkboxes that were not working and now they are working. I copied a checkbox that was working instead of creating a new checkbox.

Hi David,

Have you considered using radio button (that can be formatted to look like check boxes) instead of actual check boxes? One of my colleagues has quite a bit of experience using WF to populate PDF forms, and this is his recommendation.

EDIT: To clarify, each of these radio buttons would be in its own group, allowing the end user to select as many they like, just like with check boxes.
