This was identified when a customer upgraded from IOS Version 10.0.1 to 10.0.3.X on their iPhone and iPad.
If after upgrading an IOS device, (iphone/Ipad), that is using Offline Forms with the Laserfiche App and the Forms look ups stop working after the upgrade
Perform the following
1. While offline, remove the offline forms
2. Go to Settings in the app and select "Clear unused lookup tables"
3. Re-log into Forms in the app and reload the forms. While still online, confirm that the offline forms are downloaded.
4. Log out and check the offline form look ups
