I am trying to attach a database for Forms but this error is showing.

Hi Chase,

To attach a database to Forms, the database need to be either empty, or contain valid Forms schema.

To check the schema correct, check if the database has a table [cf_options].

Can you confirm if the database is valid?

Thank you I was able to attach the original DB and it worked.  But data sources is missing:

Would there be a reason for a demo version of Rio to not allow for lookups?

It looks like you are using Forms Essentials which does not support lookup rule, see https://support.laserfiche.com/kb/1013852/list-of-changes-for-laserfiche-forms-10-2-1
