Did you take a look at the error log, either through Laserfiche Forms instance monitoring or in the Windows Event Viewer? You'll usually find more details there.
As Anita said, the monitoring in Forms and the Windows Event Viewer will help narrow down the actual source of the problem.
If you expand the error log entry it provides a lot more detail that is usually enough to figure out exactly what went wrong.
As an example,
In my experience, the two most common reasons for Save to Repository to fail are bad credentials/permissions, or problems with the metadata fields (too long for the target field, not providing a template for a required field, etc.)
I got this same error. What I did was check to see my volume was selected within the save to repository activity.
