Form Submission Confirmed to User, Not Actually Submitted

I have a long, paginated form (employee evaluation) that my users are attempting to submit, and I have had three failures out of 7 attempts.  There are errors in the Event Viewer which I believe are related to these submissions that state "The requested service (lRoutingEngineService) may not be running."  I don't understand what would cause this to happen intermittently, and it is frustrating for the users to have to completely start over again when they received a confirmation saying the submission was successful.  After searching this topic I tried restarting the Forms Routing Service, but that did not help.  The tasks still show in Forms as assigned to the user for submission; however, the tasks no longer appear in their task in-boxes (or in their drafts), so they can't access them anymore.  This is a form that is initiated by Workflow and their prior year's goals are pre-filled to cut down on the time it takes them to complete the evaluation.  

Any help that can be offered would be appreciated.

UPDATE:  Now I have situations where the manager has made comments and routed the form back to the user; however, the system still says the form is assigned to the manager even though they can't access it at all.  This is a huge problem for us.

Hi Melanie,

Can you open a support case for this issue? It's hard to inspect routing service errors on answers forum.  Sorry for the inconvenience.

I've actually been asking my VAR for a support case to be opened since 6/22.  :(
