How to set default values in every instance of a multi-value field?

Has anyone found a way to add notes/comments to a template that auto populates the Date/Time and UserName.?  The field has to be a “multi-value” so they can continue adding comments but every time they add a comment it needs to automatically enter the Date/Time and UserName at the beginning of the note.  
Using tokens as the default value only works for the initial note but I need it to auto populate the default values for every Note.

Appreciate any suggestions.

It is a quick and ugly workflow but it works.  In the shot below where I have TestComment you would use whatever your field is named.  Put a starting conditional rule of document changed.  This workflow will look for a pattern in the last comment field.  If it doesn't match the pattern it will add date/time and initiator.

Thank you Jennifer!!  This came close to working with the only issue being an error if the user attempts to enter a comment after the template is opened.   The workflow takes control not allowing the user to add additional comments.

I would look at your starting rules.  I didn't get that error when I entered a comment right away.  The issue I had was if the comment was entered the first time the document is created, it didn't add the user and date.
