The admin password to sign into Laserfiche Forms has been lost, is there any way to get it reset?
yes, you can open the Forms Database and look for a table "cf_users".
The password is not saved in the Forms database actually. The user's password is managed through Laserfiche repository, if you have other users that have manage trustee privilege, the you can sign in the repository with that user from Laserfiche Administration Console and reset the password.

You need to be able to access the database of the repository, go to trustee table and find the user and change the value of "password" column to blank. After that you can login with no password.
