Forms- Javascript to display a fixed number of rows in table horizontally instead of vertically

Does anyone know if it is possible to to display a fixed number of rows in table horizontally instead of vertically in Forms?

Hello Ege,

Below is a pic of how I would like the document to look. The trouble comes when I need the information to fill in the template field. I have the template field set up as a multi line field but it will only capture the first column in the table.

 

The only way I can get it to store the information for each column is if I stack them by reducing it to one column and having the submitter add additional rows. As shown below.

This makes the form very long. I there a way to make it horizontal when I click on "Add" to where it will add to the right instead of underneath?
