Is it possible to have forms checkbox choices that would display a text box similar to what happens with the "Other" choice? Is this something that we can do with CSS?

You want every option to be able to change by the end user? What is the use case of that?

What I would like is a text field that would allow the user to input data. I've attached a screen snip showing my check boxes - if a user selected one or the other then they could input data to put a value on the choice. The field doesn't need to be anything fancy, just to work like the "other" choice does.
