Users at my institution are reporting problems when trying to separate and create new documents from documents while scanning using the Standard Scan mode in Web Access. We are currently using  Laserfiche Rio 10.1. I have had the users scan in black and white instead of using color or grayscale to troubleshoot a cause of the issue. I do not think it is a Laserfiche system related issue. I guessing its more or less related to the individual user's machine.
