Lookup Rule Not Populating Collection 10.2

I have a simple rule that fills a dropdown with a number. That is working ok. Then I have another rule to fill in 4 single-line fields in a collection based on the selection in the dropdown. That is not working. I pulled up another customer with a similar rule and that is working just fine. They are both on 10.2 any help would be appreciated. I'm sure it's something simple I'm missing.

Recreated the table and now it's looking up correctly. Not sure what happened.

Update:

If I leave out the when condition it fills in the fields and adds new sets. So the actual lookup rule is working correctly. The issue seems to be with the when condition. I'm including screenshots of the fields.
