update word document not working

update word document is not working, I did have this working, but I made some changes (not to the word document) and now I can't get it working again. I am starting over with a test workflow and still can't get it to work. Can someone please take a look as to what I am missing here. I did re-read "Updating a Microsoft Word Document with Laserfiche Workflow" white paper and believe I am following all instructions. Any help would be appreciated! Thanks Wendi

The workflow does everything down to Create Entry

I edited the word doc with quick parts / field / merge field. (screen shot attached)
