Missing Some Data When Form is Saved to Laserfiche

Created a process with multiple forms. Ultimately, the Doc Control Admin (DCA) is the last one to review and approve before the form is saved to Laserfiche. Here's what the DCA saw:

My data is missing. When I review the variables, it didn't keep the data prior to saving to Laserfiche:

I have no Field or Lookup Rules associated with these fields, nor any JavaScript. Here's the variable information:

The issue had to do with my fields being read only. Instead of checking the box, had to set read only in javascript. All set now.

Hi Gloria,

Which version of Forms are you using? I tried on 9.2.1 and no data are lost.

Can you open a support case and provide your business process if available? Thanks.
