The question is, how does the Connector manage to do this?  It turns out the Connector creates the document first, then adds metadata, and then opens the scanning window using the option (i.e. ScanOption) to add pages to an existing document, using the ToC ID of the document it just created.  All of these steps need to take place anyway, but the sequence is not as intuitive as you might expect.
