Late last week I tried to upgrade Forms and the Repository (Rio) to version 10.1.  We were successful with the repository but ran into an error with Forms, so Forms has stayed at v9.1.1.  I was told that Forms 9.1.1 will work with the 10.1 repository. 

But when the Forms workflow tries to upload the form and attachments to the repository the new folder (for the documents) is created but the form and attachments are not uploaded.  When I look at the status of the forms business process it says the upload is suspended.  When I retry the upload nothing happens.

I have tested the Profile for the form and it successfully connects to the repository.  Does anyone have an idea what is going on.  We are trying to upgrade Forms to 10.1 but are still running into a problem so I would like to have this fixed in the mean time.
