Email attribute in Forms

My email notifications are failing in Forms- this didn't happen until I started using Teams with a filter. Has anyone else ever experienced this?

You mean the email notification for a user task that using team with a filter? Can you check whether there is any error logged in the event log of the Forms machine(Event Viewer->Applications and Services Logs->LFForms)
