Forms database lookup runs in preview mode only

Good morning. I created a simple SQL database lookup in my form that will display the customer name when a quote number is entered. It works well under preview mode. However, when I start a process, nothing occurs. No lookup.

Oddly enough, when I created a copy of this process, it will run the lookup under both preview and live start a process.

Any suggestions where to look?

Please make sure the starting form configured in your message start event is the same form where you have the lookup rules configured.
