There might be more details in the corresponding entry in the event log. Find the LFForms event log under "Applications and Services" in Windows Event Viewer on the Forms server machine.

If this is unavailable to you, you can try some basic troubleshooting: does this happen with every file you try to upload or only certain ones? Experiment with different file sizes, types, and names. Also check if you get different results with a different File Upload field in the same form, or with a File Upload field all by itself in a brand-new test process.
