Lookup Rules not showing the information on Forms and the user is not able to see the process

Hi all,

I have created a Laserfiche Leave Request Form. Once the user is applying for a leave, I want to show the information of the Supervisor's name and email. I have created Lookup rules to fetch data in the SQL table. But it is not showing the information, can you please advise how to show this information? Please refer to attached screenshot.

 

Also, when the user is connected to the Form, he is not able to see the process. I have enabled access rights as a Submitter for this user but still it is not showing the details. Please advise.

Do you have any Field Rules set that apply to those particular fields?

Has the process been published? The user is not able to see the process from the start dialog? How about give the process URL for the user to access directly?
