Turn Collection Group Values into Multi-Value Token & Append with db Lookup Value

I have a Form Collection with two fields...TYPE and COMMENTS.

 

Ultimately, I need a multi-value token that will look like this:

NOTE:  I'm pulling in "standard comments" from a database to be used to describe the type of violation(s).

 

[Value obtained from “Query Data” activity using ADD10 to join]  [Address comments here.]

[Value obtained from “Query Data” activity using FNC10 to join]  [Fence comments here.]

[Value obtained from “Query Data” activity using G&W10 to join]  [Grass & weeds comments here.]

[Value obtained from “Query Data” activity using UNM10 to join]  [Unsightly comments here.]

 

NOTE:  The number of violations per Form will vary from case to case.  In this example, there are four.

 

I will need to merge all concatenated values of the multi-value token into a Word doc letter template.

FOR EXAMPLE:

(1) [ADD10] merge

...will appear as...

Your home must have street numbers marked in the front, and also in the back when an alley is provided.  Address comments here.

(2) [FNC10]

...will appear as...

Please repair or replace sections of fence that have missing, broken, leaning or rotten pickets. Fence comments here.

...etc...

Is this possible?  If so, how?

Hi Laura

Yes, this can be done. Your Form Workflow will need to call a Laserfiche Workflow where you can merge the Multi-Token values into a table in a Word Document set up as a template. I've attached a document below for you to start with
