CheckBox auto-population froms Text fields

Hello everyone, 

We have 2 fields in a collections. First Name and Last Name. I have a checkbox field outside the collection.

The requirements are that when one fills the first name and last name, the first item in the checkbox shows both the first name and last name. When the user click on add and fill another first name and last name field, another item in the check box appear showing the first name and last name. It should look like the screenshot below.

You've probably thought of this already but could you add it in Javascript?

But using the checkbox functionality or styles?

Were you able to accomplish this? I need to do something similar. I am not skilled at coding, but the response looks like it is limited, and I need it to adapt to a collection.

I have a collection where the user enters the names of several people. Later on, I need to populate a checkbox field with these names.

Choice one and two should have the names from the previous field.

Appreciate any help you can offer...
